John has a private practice in Oregon’s Southern Willamette Valley.

All treatment sessions are available by appointment in downtown Eugene, Oregon. The clinic is located at The Resonance Building, 840 Lawrence St., Eugene OR 97401.

Before your first appointment, please print and fill in the patient health history form before below. You may bring your completed form with you to your appointment or send a copy by email.

Billing For Services

John does not bill insurance directly, but he does provide every patient with a Super Bill which is an itemized invoice with all of the proper diagnostic and treatment codes. You may submit these Super Bills to their insurance companies for direct reimbursement.

  • Payment is due at the time of services
  • John does not accept insurance, but if your insurance covers acupuncture, he can provide you with a Superbill invoice for submission and reimbursement. A Superbill is an itemized invoice that includes the appropriate codes for your ailments being treated.
  • Patients claim that John’s fees are quite fair, and his goal is not to break your bank, but to provide you with quality healthcare for a reasonable exchange.
  • If your insurance plan does not cover acupuncture but does in fact cover Dry Needling and you have been referred to the clinic for Dry Needling, then your Super Bill will have appropriate Dry Needling codes checked off for you to present to your insurance provider.  The referring physicians and insurance companies should be well aware that Dry Needling is absolutely in the Scope of Practice for all acupuncturists.

Cancellation Policy

John understands that life happens—but to be respectful of everyone’s time, we kindly ask for at least 24 hours’ notice if you need to cancel or reschedule. Cancellations made with less than 24 hours’ notice may be subject to a fee.
